Hospital discharge is one of the most common reasons a family calls DayLight for the first time. The patient is ready to leave, the bed is needed, and the drive home or to a rehab facility has to be safe, on time, and matched to the patient’s mobility. This guide walks caregivers through the process.
Which hospitals we pick up from
DayLight handles same-day discharge rides from every major San Diego County hospital. That includes Sharp Grossmont in La Mesa, Sharp Memorial in Serra Mesa, Sharp Chula Vista, Scripps Mercy in Hillcrest, Scripps Mercy Chula Vista, Scripps La Jolla, Scripps Green, UCSD La Jolla (Jacobs and Thornton), UCSD Hillcrest, Kaiser Zion, Kaiser San Diego, VA San Diego Healthcare System (La Jolla), Palomar Escondido, and Paradise Valley in National City.
How the booking flow usually works
A case manager, social worker, or family member calls dispatch from the bedside. We confirm within minutes whether we can stage a vehicle, give a realistic pickup window, and match the vehicle to the mobility level (wheelchair, ambulatory, or gurney). Most San Diego County hospital pickups can be staged within 60 to 90 minutes when availability allows, sometimes faster.
We coordinate with the discharge lounge or loading zone. The driver meets the patient at the designated discharge door, handles any small bags, and confirms the drop-off before leaving.
Special considerations for hospital pickups
Some patients leave the hospital with portable oxygen, IV poles, or temporary equipment. Tell dispatch at booking so the right vehicle arrives. Wheelchairs are often provided by the hospital; we pick up with the hospital chair and the patient transfers to their own chair or bed on arrival. For some discharges, a walker or cane is all that is needed, in which case ambulatory service is the right level.
If the patient is going to a skilled nursing facility, rehab, or assisted living rather than home, we coordinate the arrival time with the receiving facility so there is no waiting around. This matters because SNFs often require a specific arrival window to process admission paperwork.
Insurance and payment at discharge
Many discharge rides are covered by the patient’s insurance. Medi-Cal members usually have NEMT coverage if the ride is medically necessary and authorized. Medicare Advantage plans with a transportation supplemental benefit often cover the ride home. VA patients discharging from VA La Jolla are usually covered under VA Community Care.
Private pay is also common because discharge rides are time-sensitive. If there is no time to sort out broker authorization, families pay directly and handle reimbursement later. DayLight quotes flat base plus mileage so the cost is predictable.
Working with discharge planners and case managers
Discharge planners at Sharp Grossmont, Scripps, UCSD, and Kaiser reach out to DayLight every week. We keep a direct dispatch line for facility partners and confirm pickups faster than a broker routing flow would allow. If you are a case manager reading this, call dispatch and ask for the facility-partner packet.
